Registration New Patients
If you live within our practice area, register with us by speaking to our reception team.
To find out if you live within our practice catchment area, view the map.
They will be happy to guide you through the process. To register, you will need your photo ID (passport or driving licence) and proof of address (such as a bank statement or gas/electricity bill).
You will need to complete a registration form (GMS1) and a health questionnaire, which can be collected from our reception.
The questionnaire provides useful information to our GPs while we await your full registration.
When you register, you will be offered a health check with a member of our healthcare team.
This provides us with up-to-date information about your health status and allows us to take accurate note of any repeat prescriptions you may need.
Medical treatment is available from the date of registration. Please contact our reception for further information.
All registered patients are assigned one named GP, who is accountable for their care.
